In the ambulance
EMS teams assess the patient, address immediate life threats, provide emergency care, work to stabilize the person, and transport them safely. They must act with the information available at that moment.
One scan. The information you choose.
My Alert Band helps make emergency contacts, medical notes, caregiver instructions, child safety details, travel information, and pet identification easier to access with a simple scan.

Why it matters
If a person cannot clearly communicate, first responders may have limited access to medical history, medications, allergies, emergency contacts, or caregiver instructions.
EMS teams assess the patient, address immediate life threats, provide emergency care, work to stabilize the person, and transport them safely. They must act with the information available at that moment.
The hospital team continues urgent care while gathering history, examining the patient, considering possible causes, and ordering appropriate tests. Treatment may begin before every answer is known.
A scannable MAB profile is designed to make the information you chose easier to find—such as allergies, medications, conditions, contacts, and care notes—when you may not be able to explain it yourself.
Watch: Why it matters
See how My Alert Band can help make the safety information you choose easier to find when speaking for yourself may not be possible.

Traditional engraved ID
A name, condition, phone number, or allergy may fit. A longer medication list, several contacts, detailed care instructions, or changing information may not.
QR-connected MAB profile
A scan can make selected medications, allergies, conditions, contacts, caregiver notes, and other useful details easier to find—and the information can be updated when life changes.
MAB does not replace medical identification, professional judgment, emergency services, or direct communication. It cannot guarantee that a responder will scan it or that information will change treatment.
The information you choose
The device number is required for setup. Every profile detail below is optional, and you can update your choices later.
How you want to be identified.
Family, caregiver, or another trusted person.
Food, medication, environmental, or other allergies.
Names, doses, schedules, and important notes.
Diagnoses or conditions you want made available.
Communication, mobility, sensory, special-needs, caregiver, travel, or safety notes.
Names and contact details for relevant providers.
Only the plan details you choose to make available.
If known and if you choose to include it.
Preferred language and helpful communication notes.
Living will, advance directive, DNR, religious directive, or other document you choose to make available.
X-ray/radiography, MRI, CT, ultrasound/sonography, PET/SPECT nuclear medicine, mammography, fluoroscopy, echocardiography, and other records you choose.
Additional information you choose to share that does not fit into another category.
A current photo, age, height, identifying marks, or other details you choose to make available.
Pacemaker, insulin pump, shunt, port, prosthetic, oxygen, mobility aid, or other equipment you choose to list.
Simple by design
Unlike a traditional engraved ID with limited space, a QR-connected profile can make more of the information you choose available. Set it up once, update it when needed, and keep the product with the person or pet it protects.
